Your initial consultation is an opportunity for you and our Dental Implant and Periodontal Team to get to know one another.
For you, it provides a chance to get to know us and ask any questions so that you feel you’re in the best possible hands for your dental implant or periodontal treatment, and ensure that you are provided all the information to make an informed decision.
For us, it’s an opportunity to gain a comprehensive understanding of your medical and dental history and your dental implant and periodontal needs. Through a detailed discussion and a thorough examination, photographs, x-rays and any other necessary diagnostic tests our team can gather all the necessary information to advise on the best treatment plan for you.
We will then discuss all the options available before formalising them in a personalised treatment plan so that you know the steps and costs for each stage of your treatment.
As a new patient at our practice we have a lot to catch up on. For this reason, we will request that you complete a New Patient Medical and Dental History Form, detailing your medical and dental history. For your convenience, we have made this form available for you to download and complete in your own time. Simply fax or email it back to us or bring it to your first appointment. However, if you don't get around to doing this you can complete the form in just a few minutes upon arriving at our practice.
